Premier Property Management Group

I/We (Name), (Social Security #) of (Address), Owner(s) or Agent(s) of the Rental Unit(s)at (Service Address), hereby request that UGI Utilities, Inc (UGI) provide continuous gas service to the above unit(s) during periods of vacancy subject to the following terms and conditions:

1. The Owner/Agent authorizes UGI to establish accounts in the name of the Owner/Agent upon the issuing of a final bill to a prior tenant or upon a notice of a vacancy from the Owner/Agent or a tenant in any unit.
2. There will be no intiation charge to the Owner/Agent for the establishment of accounts pursant to the Agreement.
3. In order to remain on this agreement, the Owner/Agent must pay all bills, on such accounts on or before the due date stated on each bill. Late payments will result in removal from the program. Bills and notices will be addressed to the Owner/Agent at:
   Premier Property Management Group PO Box 6127 Harrisburg, PA 17112 (Mailing Address)
   until changed by written notice. The Owner/Agent shall notify UGI in writing within five days of any change in ownership of the property or change of mailing address
4. This Agreement intends to provide continuous gas service to affected units only while unoccupied and not affect any such unit's account has been:
   1. Terminated by UGI per applicable regulations, or
   2. discontinued by a tenant who wished to remain on the premises.
   In either of these instances, the Owner/Agent shall contact the tenant for additional information, if necessary.
5. The Owner/Agent shall remain responsible for bills rendered to the account for an affected unit until the effective date of service to a subsequent tenant of any such unit or the transfer to a new owner.
6. The Owner/Agent, upon request, shall provide to UGI the forwarding address, if any, of the prior tenant of any unit covered under the Agreement.
7. This Agreement, subject to applicable provisions of UGI's duly filed tariff, constitutes the entire contract between the parties with respect to the subject matter herein, and shall not be amended, modified or changed without prior written approval of UGI.
8. The undersigned, intending to be legally bound, represents and warrants that he is authorized to and does enter into this Agreement for the Owner/Agent.
